Privacy Policy & Patient Rights

This page explains your rights as a patient at Barbell Rx Chiropractic & Rehab: how to get your records, who licenses us, how to raise a concern, what a Good Faith Estimate is, and how we protect your health information. If you have a question about any of it, call (940) 461-8880 or email coby@barbellrxchiro.com.

Your Patient Rights in Texas

Texas law (Health and Safety Code §181.105) requires every clinic to explain how to request your records, contact the board that licenses us, and file a complaint. Here's how.

How to request your health records

You have the right to see and get copies of your records at any time. To make a request:

  1. Send us a signed, dated request. Written requests are best, but you can also ask at your visit and we'll help you fill one out.
  2. Tell us which records you need, for example "all records from January 2027 to now."
  3. Include your full name, date of birth, and a phone number or email so we can confirm it's you.
  4. Send it by email to coby@barbellrxchiro.com, or by mail or in person to 1910 Avenue G NW, Childress, TX 79201. You can also call (940) 461-8880.

We'll get your records to you within 15 business days. A reasonable, cost-based fee may apply for copies, within the limits Texas sets. If you want your records sent to someone else, like another doctor, you'll sign a short release form.

How to contact our licensing board

Dr. Coby Koehler, DC, is licensed and regulated by the Texas Board of Chiropractic Examiners (TBCE). If you have a question or concern about a chiropractor's license or professional conduct, you can contact the board directly:

How to file a complaint

If something didn't go right, we'd like to hear about it first. Call (940) 461-8880 or email coby@barbellrxchiro.com.

You can also file a complaint with any of these agencies:

Filing a complaint will never affect your ability to get care here.

Your Right to a Good Faith Estimate

If you don't have insurance or aren't using it, you have the right to a written Good Faith Estimate of what your care will cost. You'll get it at least 1 business day before your visit, and you can ask for one before you book. If your bill ends up at least $400 more than your estimate, you can dispute it, so keep a copy. To learn more, visit www.cms.gov/nosurprises/consumers or call 1-800-985-3059.

Language Help and Accessibility

Free language help is available, and we can provide information in other formats, like large print, at no cost. Call (940) 461-8880 (TTY: 711). We don't discriminate on the basis of race, color, national origin, age, disability or sex.

Notice of Privacy Practices

Effective November 9, 2026

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Your rights

When it comes to your health information, you have the right to:

  • Get a copy of your record. Ask in writing, and we'll send a paper or electronic copy within 15 business days. A reasonable, cost-based fee may apply.
  • Ask us to correct your record. If we say no, we'll explain why in writing within 60 days.
  • Choose how we contact you, for example only on your cell phone, or at a different address. We'll say yes to reasonable requests.
  • Ask us to limit what we use or share. We don't have to agree, and if we do, we may still share the information if you need emergency care. If you pay for a service in full yourself, you can ask us not to share it with your health insurer, and we'll say yes unless the law requires otherwise.
  • Get a list of who we've shared your information with over the past six years, other than for treatment, payment, running the clinic, or sharing you asked for. One list a year is free.
  • Get a paper copy of this notice at any time.
  • Have someone act for you, like a person with your medical power of attorney or your legal guardian.
  • File a complaint with us, or with the U.S. Department of Health and Human Services, Office for Civil Rights, at hhs.gov/hipaa/filing-a-complaint or 1-877-696-6775. We will never hold a complaint against you.

How we use and share your information

We use and share your information to:

  • Treat you, for example by sending an update to the doctor who referred you.
  • Get paid, for example by sending a Medicare claim for a covered adjustment.
  • Run the clinic, for example to send appointment reminders and review our care.

We use a few outside companies to run the clinic, such as our scheduling and records software, phone system, secure email, home-exercise app and card processing. Each one signs an agreement to protect your information. They may combine information to provide their service, or remove anything that identifies you and use what's left, as the law allows.

We share information with family or friends only if you name them on our contact form or don't object, and in a disaster.

We may also share information when the law allows or requires it: for public health and safety (like product recalls or reporting abuse or neglect), health oversight such as the Texas Board of Chiropractic Examiners, law enforcement, court orders and subpoenas, workers' compensation, coroners and organ donation, special government functions, and to prevent a serious threat to someone's health or safety. We don't do research or fundraising.

Anything else needs your written permission. That includes marketing and selling your information, which we never do. You can take your permission back in writing at any time.

Texas law gives you extra protection: we'll get your written permission before sharing your information electronically for anything other than treatment, payment, running the clinic, or as the law otherwise allows.

If we ever have substance use disorder records protected by federal law (42 CFR Part 2), we won't use them against you in any investigation or proceeding without your written consent, or a court order and a subpoena.

Our duties

We're required by law to protect your health information, follow this notice, give you a copy of it, and let you know promptly if a breach affects your information. We can change this notice, and any change will apply to all the information we have about you. The current version is always on our website, in the clinic, and available on request.
